TO PROVIDE A SPECIALIST PALLIATIVE CARE SERVICE FOR PATIENTS WITH SPECIFIC LIFE-LIMITING ILLNESSES LIVING IN THE AREA COVERED BY WREXHAM HOSPITALS AND TO SUPPORT THEIR FAMILIES IN A CARING ENVIRONMENT. THE ACTIVITIES WILL BE UNDERTAKEN BY A SPECIALIST TRAINED MULTI-DISCIPLINARY TEAM WHO WILL PROVIDE SYMPTOM CONTROL, ASSESSMENT, TERMINAL CARE, ONGOING EMOTIONAL AND BEREAVEMENT SUPPORT + EDUCATION
